LC-MS/MS has become an alternative for steroid analysis in clinical routine due to simplified sample preparation and increased specificity and sensitivity compared to immunoassay. Also, it permits simultaneous measurement of several steroids which opens novel diagnostic possibilities. With our recently developed method for simultaneous quantification of 17α-hydroxyprogesterone, aldosterone, androstenedione, cortisol, cortisone, dehydroepiandrosterone sulfate (DHEAS), estradiol, progesterone, and testosterone combining online solid phase extraction (SPE) with LC MS/MS or LC MS3 we started 2016 in clinical routine diagnostics replacing immunoassays for serum, urine, and saliva with one analytical LC-MS/MS method. The method with a total run time of 5 min is applicable for 20 μL of human plasma/serum, 100 μl saliva, urine or 10-20 mg of hair. In complex sample matrices like hair MS3 detection was found to be appropriate for quantitation. The method is now analyzing more than 100 samples per day. In this presentation, experience from the implementation of our in-house assay as IH-IVD in an accredited laboratory is presented.
